Benefits Eligibility *Full-time employees are eligible for health insurance benefits the first of the month following a 6-month waiting period from employee hire date. *Full-time employees are eligible for the 401K Retirement Plan with Company Match on enrollment periods of January or July after one year of full-time employment from employee hire date. *Eligible full-time employees must complete and submit correct enrollment forms on or before the enrollment deadline for coverage to be effective. Types of Health Insurance OfferedMedical Insurance: Plan option is an “open access” National HMO Dental Insurance: Plan options include one DMO and one PPO Vision Insurance: PPO plan only Life Insurance: 100% paid by the company; benefit equal to one times the employee's annual salary Supplemental Life Insurance: 100% paid by the employee Long Term Disability: 100% paid by the company 401K Retirement Plan with company match *30%-73% of premiums are paid by the company Other Employee BenefitsFull-time employees are eligible to accrue vacation for each calendar month of service from their date of hire. The vacation accrual rate is based on your length of employment from the previous year, as follows:
Vacation Sell Back Plan The company believes that employees should have the benefit to sell back their vacation to the company rather than take vacation if they wish. Requests may not exceed 40 hours/5 days. There are eligibility requirements. This is a discretionary plan. Holiday PolicyFull-time employees are eligible for Holiday pay after completing the 90-day Introductory Period. Recognized Holidays are as follows:
Education Assistance Plan Policy It is the company's belief that assisting full-time employees in the pursuit of education and skills training that directly benefits their current work-related knowledge base will benefit both the employee and the company. There are eligibility and reimbursement requirements. This is a discretionary plan. Employee Referral BonusThe company believes in rewarding current full-time employees for referring a new employee to the company. The referral bonus for eligible salaried positions is $100. The referral bonus for eligible hourly positions is $50. In order to qualify for the referral bonus, the referring employee must be able to confirm to HR that he/she is the INITIAL source of the hire and the referral must be employed for 6 months. The referring employee must be employed to receive the benefit. There are eligibility requirements. This is a discretionary plan.
